My health for life is a Queensland Government funded program lead by Diabetes Queensland in partnership with the Heart and Stroke Foundations, Queensland Primary Health Network, Ethnic Communities Council of Queensland and Queensland Aboriginal and Islander Heath Council.
My health for life is a program designed to help Queenslanders live healthier, happier lives by improving their health and reducing their risk of developing chronic conditions like type two diabetes, heart disease and stroke.
My health for life recognises that everyone is different and that changing habits to improve our health takes time and support. My health for life is a research based, behaviour change program which helps participants gain knowledge along with the skills and support to achieve their goals. My health for life health coaches are trained health professionals who work in partnership with participants, either through one-on-one phone coaching or small group sessions in local communities.
